This Consumer Health Data Privacy Policy describes how Mystic Rose: Grief Support & Spiritual Care collects, uses, and shares "Consumer Health Data" as defined by the Washington My Health My Data Act. This policy applies to Washington residents and individuals whose data is collected in Washington.

2. Categories of Consumer Health Data Collected

In providing intuitive grief support and thanatology services, the following categories of data may be collected:

• Individual Health Conditions and Status: Information you share regarding your emotional well-being, grief process, and mental or physical health status related to loss.

• Sensitive Personal Information: Information regarding your spiritual beliefs or religious affiliations as they relate to your grief journey.

• Contact Information: Data collected via website forms (e.g., name, email) that may be linked to your health-related inquiries.

3. Sources of Collection

This data is collected directly from you when you:

• Complete a contact or intake form on our website.

• Engage in intuitive grief support sessions.

• Communicate with us via email or phone.

4. Purpose of Collection and Use

Your Consumer Health Data is collected and used for the following purposes:

• To provide requested intuitive grief support and thanatology services.

• To personalize your spiritual guidance based on your unique grief experience.

• To maintain professional records in accordance with the ADEC Code of Ethics.

• Note: This data is not used for marketing or profiling.

5. Sharing of Consumer Health Data

Your health data is not "sold". Your health data is only shared in the following limited circumstances:

• Service Processors: With third-party vendors (e.g., secure booking or payment platforms) that are contractually bound to process data only to provide the service you requested.

• Legal Requirements: If required to do so by law or to protect the safety of yourself or others (e.g., mandatory reporting).

6. Your Privacy Rights

Under the MHMDA, you have the right to:

• Confirm and Access: Verify if we are processing your data and request a copy.

• Withdraw Consent: Withdraw your consent for future data collection or sharing at any time.

7. Right to Withdraw Consent and Request Deletion

You have the right to withdraw your consent for the collection and sharing of your consumer health data at any time. You also have the right to request that we delete all consumer health data we have collected from or about you.

How to Exercise Your Rights:

To withdraw consent or request deletion, please email drmonique@mysticrose.us with the subject line "Privacy Request." We will authenticate your request and respond within 45 days as required by Washington law.

Exceptions to Deletion:

Please note that while we will delete your health data from our active marketing and operational systems, we may retain certain records to comply with professional ethics under the ADEC Code of Ethics, to complete a transaction you requested, or to comply with legal obligations under RCW 18.19.060.
